How to register

If you live within the Sutton or Merton boroughs you can register with the GP surgery at the Wilson Health Centre. If you live outside the boroughs of Sutton and Merton, we will deal with your requests on an individual basis.

You can complete the first part of the register online now or just pop into reception and we will talk you through the process. Once registered, we can request your medical records from your previous practice via NHS Sutton and Merton.

Please remember to notify us if you change your name, address or telephone number. You can notify us online, by fax or post, or just pop in and inform reception.
